This rapid living systematic review outlines studies proposing care models for post-COVID conditions. Authors suggest that a complete care model should integrate primary care, rehabilitation services and specialized clinics for medical assessment, with potential entry ways via a centralized referral system. An integrated care model for post-COVID condition is illustrated in the review.
